Hospice size exemption forms are now available

The Hospice CAHPS survey participation exemption for size request form is now available for the 2019 calendar year.

Hospices that served fewer than 50 survey-eligible decedents/caregivers in 2018 can apply for an exemption from participation in the survey in 2019.

The exemption is only good for one year, so hospices that submitted a form in a prior year must reapply.
