CMS has identified an issue with the pricer that is impacting some 2025 claims, according to Medicare Administrative Contractors in an alert to providers Feb. 25.
Agencies need to ensure they are reporting the correct county code (CBSA) on their claims, notes Palmetto GBA.
Some 2025 claims are returning with reason code 37257, incorrectly noting an invalid or missing county code. This is occurring for claims that changed from an urban CBSA to a rural CBSA with the 2025 final rule, Palmetto GBA states.
CGS notes that a new pricer will be released to correct the issue.
