An Oklahoma-based home health agency has reported a hacking incident that involved the protected health information of 4,743 individuals, according to the Department of Health and Human Services’ (HHS) breach report.
Reported March 19, 2025, the data breach is one of more than 30 hacking incidents submitted in the month of March, according to HHS.
The HIPAA Journal reports that Millennium Home Health Care of Tulsa, Okla., discovered unauthorized access to two of its computer servers in January 2025. Information accessed included names, birth dates, addresses, phone numbers, Social Security numbers and medical information.
