Mistakes associated with overlapping periods account for the top error that one Medicare Administrative Contractor is seeing in the first weeks of 2025.
CGS recently released its top claim submission errors in January and February of 2025. At No. 1: “From date on Notice of Admission (NOA) falls within an existing home health admission period.”
This error tends to happen when there is an existing open admission period from a different home health agency, CGS notes. Other reasons include a duplicate NOA submitted and a new NOA is submitted when a patient is readmitted to the same agency in the same 30-day period as a discharge due to an admission to a hospital or SNF.
The top five denial reasons are listed below:
1. From date on NOA falls w/in an existing HH admission period: 5,556
2. Services w/in HMO Period and no Hospice involvement or services not w/in Hospice period: 3,624
3. No OASIS assessment found: 3,185
4. RC will be assigned when a HH TOB 3X9 was entered with an invalid CBSA code: 2,800
5. Statement From Date is on or after 01/01/2022 and less than 24 months from claim Admit Date and a matching Home Health NOA cannot be found: 2,753
