Providers, patients and referral sources have fresh publicly reported data on how home health agencies fared on OASIS and HHCAHPS measures.
 
CMS posted the quarterly update to Care Compare on April, 23, 2025. New data includes HHCAHPS results from October 2023 to September 2024 and OASIS data from July 2023 through June 2024.
 
Nationally, agencies continued to see improvement on several OASIS measures, including:
  • How often patients got better at walking or moving around: 87.7% (up from 87.5% in January and 86.1% in April 2024).
  • How often patients got better at getting in and out of bed: 88.7% (up from 88.4% in January and 87.1% in April 2024).
  • How often patients got better at bathing: 89.6% (up from 89.3% in January and 88.0% in April 2024).
  • How often patients’ breathing improved: 90.0% (up from 89.7% in January and 88.3% in April 2024).
  • How often patient got better at taking their drugs correctly by mouth: 86.4% (up from 86.0% in January and 84.2% in April 2024).
The Discharge Function Score national average climbed to 67.26%, up from 67.15% in January. The new Transfer of Health Information measures also saw improvement, with Transfer to Provider at 80.97% (previously 80.13%) and Transfer to Patient at 90.19% (previously at 89.57%).
 
Among the notable exceptions were drops in how often the home health team determined the patient’s flu vaccine status, now down to 67.5%, compared to 72.8% in April 2024. How often physician-recommended actions to address medication issues were completely timely also fell, now at 94.4% compared to 95.2% reported a year ago.
 
For HHCAHPS results, patients who reported their home health team gave care in a professional way climbed to 89%, compared to 88% in April 2024. The response rate dropped year-over-year, down to 23% from 24% reported last spring.
