Home health agencies may soon see a process measure in the quality reporting program around Advance Care Planning (ACP).
CMS announced its 2025 Measures Under Consideration List on Dec. 15. The sole item considered for the Home Health Quality Reporting Program is an ACP process measure.
A general description of the measure that could be applicable to many provider types focuses on patients with an ACP document on file or documentation of an ACP discussion.
In its justification for the measure, CMS noted that engagement in ACP remains low across the U.S. A 2023 study of older adults found that 42% reported conversations with surrogates about advanced care planning and 15% with clinicians. Of those, less than half the conversations were rated as “detailed high-quality discussions.”
