The U.S. Office of Inspector General (OIG) released the results of a compliance audit into Texas-based home health agency Guardian Home Care on Dec. 17, 2025. The report was part of a series of nationwide audits that examined compliance with Medicare billing requirements for home health services from July 1, 2021, to June 30, 2023.  
 
During the audit period, Guardian Home Care received $35,645,778 in Medicare payments from over 17,000 claims.  
 
Of 100 randomly selected claims, only three were incorrectly billed — two related to coding and billing issues, and the third did not meet plan of care requirements. Guardian received an overpayment of $1,690 for one of these incorrectly billed claims.  
 
Based on these results, the OIG suggests that Guardian Home Care refund the overpayment to Medicare and implement internal audits to identify any similar issues.
