OASIS-based measures include results from July 2024 to June 2025, while HHCAHPS scores reflect surveys from October 2024 to September 2025. Claims-based measures are only updated once a year with the October refresh.

  • Changes in Skin Integrity post-acute care: pressure ulcer/injury: 0.24%
  • How often physician-recommended actions to address medication issues were completely timely: 94.49%
  • Percent of Residents Experiencing One or More Falls with Major Injury: 0.93%
  • Discharge Function Score: 71.04%
  • Transfer of Health Information to the Provider: 80.5%
