HHS today named 26 recipients of a special award designed to fund innovative ideas for care coordination.

The projects HHS decided to fund are widely different and funding amounts vary by project, but many of the awardees will take a community-based approach to care.

That includes, for example, a project helmed by George Washington University in Washington, D.C. which will explore the benefits of telehealth in improving patient adherence. Another project spearheaded by Boston-based Beth Israel Deaconness hospital will aim to smooth transitions into the community after a hospital stay.
