Community Health Accreditation Partner (CHAP) released its top survey deficiencies for home health and hospice in 2024, along with tips for maintaining compliance on these issues, on Jan. 28. 
 
Chief among the home health deficiencies was G574, which details the requirements for written, individualized plans of care. The top deficiency for hospices — which included tags L545, L548 and L549 — also revolved around issues with plans of care.  
 
CHAP’s tips for avoiding these issues include involving all disciplines in relevant education, focusing on each patient’s unique needs and using a Quality Assurance and Performance Improvement (QAPI) program to achieve and maintain compliance. 
 
Lists of the top 10 home care, palliative care and Durable Medical Equipment, Prosthetic Devices, Prosthetics, Orthotics and Supplies (DMEPOS) deficiencies are also available.
