An Illinois-based home health agency received more than $100,000 in overpayments due to incorrect billing, coding and documentation, according to the HHS Office of Inspector General (OIG).  
 
The audit of HRS Home Health, released July 3, 2025, found 80 of the 100 claims reviewed from 2020 and 2021 met billing requirements. The 20 other claims had errors likely due to the agency not always adequately reviewing medical record documentation to prevent the incorrect billing of Medicare claims, according to the OIG. 
 
The report states that HRS likely received at least $100,696 in overpayments related to these and other improperly billed claims submitted during the audit period. The OIG recommends that HRS refund these estimated overpayments, consider conducting internal audits and strengthen its review process to boost compliance. 
 
HRS noted in its response to the OIG’s report that it intended to repay HHS for seven of the identified errors but pushed back on the recommendation that this amount should be expanded to include other possible claims. The agency also stated that it will increase internal audits and require all claims to be reviewed for compliance.
