Results for the Discharge Function Score continue to improve nationally, according to the latest public reporting released from CMS on Care Compare.
 
CMS posted the quarterly update July 16, 2025. New data includes HHCAHPS results for all of 2024 and OASIS data from October 2023 through September 2024.
 
The national average for the Discharge Function Score climbed to 68.62% — a significant jump from 67.26% reported in April and 67.15% reported in January.
 
Nationally, agencies also continued to see improvement on several other OASIS measures, including:
  • How often patients got better at walking or moving around: 88.0% (up from 87.7% in April and 86.7% in July 2024).
  • How often patients got better at getting in and out of bed: 88.9% (up from 88.7% in April and 87.6% in July 2024).
  • How often patients got better at bathing: 89.8% (up from 89.6% in April and 88.5% in July 2024).
  • How often patients’ breathing improved: 90.3% (up from 90.0% in April and 88.8% in July 2024).
  • How often patient got better at taking their drugs correctly by mouth: 86.8% (up from 86.4% in April and 84.9% in July 2024).
The Transfer of Health Information measures also saw improvement, with the national average for Transfer to Provider at 81.62%, up from 80.97% in April. Transfer to Patient was at 90.66%, up from 90.19% in April.
 
Among the notable exceptions was a drop in how often the home health team determined the patient’s flu vaccine status, now down to 67.4%, compared to 72.6% in July 2024. How often physician-recommended actions to address medication issues were completely timely remained at 94.4%, down from 94.9% reported in July 2024. 
 
For HHCAHPS results, patients who reported their home health team gave care in a professional way climbed to 89%, compared to 88% in July 2024. The overall percent of patients who reported they would recommend the agency to friends and family also climbed one percentage point year-over-year, from 78% to 79%.
