CMS has released the Final 2025 Annual Performance Reports (APRs). Now available in iQIES, the final APRs follow the preliminary and preview reports released earlier this fall.
 
The APRs include payment adjustments for 2026. Agencies will receive an adjustment of up to 5% (higher or lower) based on their 2024 performance on key OASIS, claims and HHCAHPS survey measures.
 
They include:
OASIS-based measures
  • Discharged to Community
  • Improvement in Dyspnea
  • Improvement in Management of Oral Medications
  • Total Normalized Composite Change in Mobility (TNC Mobility)
  • Total Normalized Composite Change in Self-Care (TNC Self-Care)
Claims-based measures
  • Acute Care Hospitalization (ACH)
  • Emergency Department Use Without Hospitalization (ED Use)
HHCAHPS Survey-based measures
  • Care of Patients
  • Communication Between Providers and Patients
  • Specific Care Issues
  • Overall Rating of Home Health Care
  • Willingness to Recommend the Agency
Note: Several measures shift for your 2025 performance year, which will impact payment adjustments for 2027.
  • Discharge to Community will come from claims instead of the OASIS.
  • The OASIS composite scores are replaced with the Discharge Function Score.
  • Hospital and ED measures are replaced with the Potentially Preventable Hospitalization (PPH) measure.
Each agency's resulting payment adjustment and performance on key measures is publicly reported by CMS each January.
