Referral sources and patients have new data on home health quality after the shutdown delayed the October update of Care Compare. CMS updated the data Nov. 20, 2025.
 
OASIS and HHCAHPS measures are updated quarterly, but the fall update is the only one that refreshes key claims-based measures.
 
Notably absent: CMS didn’t update the measures Discharge Function Score or the Transfer of Health Information to the Provider. With the Care Compare update, the items are publicly reported as “Not available.”
 
Keep in mind, the January preview reports available in iQIES should have your agency scores on these measures.
 
Here’s a look at the October 2025 numbers nationally:
  • How often the home health team began their patients' care in a timely manner: 96.37%
  • How often the home health team determined whether patients received a flu shot for the current flu season: 66.18%
  • How often patients got better at walking or moving around: 88.22%
  • How often patients got better at getting in and out of bed: 89.18%
  • How often patients got better at bathing: 90.09%
  • How often patients' breathing improved: 90.62%
  • How often patients got better at taking their drugs correctly by mouth: 87.21%
  • Changes in Skin Integrity post-acute care: pressure ulcer/injury: 0.24%
  • How often physician-recommended actions to address medication issues were completely timely: 94.45%
  • Percent of Residents Experiencing One or More Falls with Major Injury: 0.93%
  • Transfer of Health Information to the Patient: 91.13%
  • Potentially Preventable Rehospitalization (post-discharge): 4.12%
  • Discharge to Community: 77.71%
  • Potentially Preventable Hospitalization (within-stay): 10.83%
