Agencies can get their first look at expected payment adjustments for 2027 under Home Health Value-Based Purchasing (HHVBP).
 
The preview 2026 Annual Performance Reports were published in the Internet Quality Improvement and Evaluation System (iQIES) on Aug. 20, 2026.
 
Results include the agency’s Annual Payment Percentage (APP) based on performance in 2025. The APP is an up to 5% payment increase or decrease based on how an agency’s results compare to similar agencies nationwide.
 
Measures used to determine agency results include:
  • The OASIS-based Improvement in Dyspnea, Improvement in Management of Oral Medications and Discharge Function Score.
  • The claims-based Discharge to Community and Home Health Within-Stay Potentially Preventable Hospitalizations.
  • The HHCAHPS survey-based Care of Patients, Communications Between Provider and Patients, Specific Care Issues, Overall Rating and Willingness to Recommend.
CMS applies the payment adjustment percentage to claims paid through traditional Medicare, but other payors (e.g., Medicare Advantage, Medicaid and managed Medicaid) can utilize the publicly-reported results to make their own payment adjustments.
 
Agencies have until Sept. 4, 2026, to submit a recalculation request.
 
Following the appeals period, the final APRs for 2027 payments will be published in iQIES later this year and released publicly in January.
