CMS released the updated Home Health Grouper Aug. 20 containing acceptable primary diagnoses and the high and low comorbidity adjustment subgroups for the new FY2027 codes.
 
Key updates effective Oct. 1, 2026:
  • New code E89.830 (Post bariatric hypoglycemia) received a MMTA – Endocrine clinical group designation and Endocrine 5 comorbidity group designation.
  • New codes I49.81 (Brugada syndrome), I49.82 (Ventricular bigeminy) and I49.89 (Other specified cardiac arrhythmias not elsewhere classified) each received a MMTA – Cardiac and Circulatory clinical group designation and Heart 10 comorbidity group designation.
  • Several new codes were also added to the list of unacceptable principal diagnoses including the new BMI codes Z68.18 (Body mass index [BMI] 18.4 or less, adult) and Z68.19 (Body mass index [BMI] 18.5-19.9, adult).
The grouper also contains a listing of code first diagnosis additions and removals, deleted diagnosis codes and modified diagnosis code descriptors.
