After hearing industry feedback, CMS plans to continue modifying a proposed clinical template for doctors to use when documenting face-to-face encounters. An updated version of the template is expected within weeks, the federal Medicare agency announced during a Feb. 11 open door forum.

CMS has been vigorous in helping providers, support vendors and clearinghouses to prepare for the Oct. 1, 2015, transition to the 10th revision of the international classification of diseases (ICD-10), the General Accountability Office (GAO) finds.
