The public is getting its first look at the home health quality measures around the Transfer of Health Information (TOH) to providers and patients. The national average is 80.13% for the TOH to subsequent providers and 89.57% for the TOH to patients. Results are based on OASIS assessments submitted from April 2023 to March 2024.
