The new year should bring fresh attention to the Discharge Function Score, as public reporting begins and value-based results are impacted.
 
The Discharge Function Score is based on responses to 12 GG items in the OASIS assessment.

  • GG0170R. Wheel 50 feet with two turns (used in place of walking activities for patients who don’t walk at both SOC/ROC and discharge)
Public reporting on the measure is expected to begin with January’s Care Compare refresh. And results will be used for the 2025 performance year of the Home Health Value-Based Purchasing (HHVBP) model, replacing the MM items for self-care and mobility.
 
CMS released a quality measure overview doc that addresses some of the top questions about the measure, including how CMS will still score patients when a GG item is skipped, dashed or coded with Activity Not Attempted.
 
“Statistical imputation is an approach that recodes missing start of care/resumption of care (SOC/ROC) and discharge GG item values to an estimated most likely value had the functional status self-care or mobility activity been assessed,” CMS explains.
