Agencies are struggling to accurately log the Notice of Admission (NOA) and OASIS assessment so that CMS can match these items with the corresponding claim, according to CGS.
 
The Medicare Administrative Contractor released its list of the top 10 claim submission errors for home health providers.
Among them: “No OASIS assessment found” and “Statement from date less than 24 hours from claim ‘admit” date and a matching NOA can’t be found.”
 
The rest of the top claim submission errors were:
  • Episode "To" date not 30 days greater than "From" date.
  • No payment can be made since the corresponding NOA was received 30 days or more after the claim FROM Date.
  • HH claim rejected. This claim had service dates that matched a claim already processed and at least one rev code was the same.
  • Duplicate claim.
  • Each line of charges on this claim has been rejected and denied.
  • A Home Health (HH) Notice of Admission (NOA) overlaps an existing HH admission.
  • The From and Through dates on HH claim are outside the HH Admission period.
  • Services within HMO Period and No Hospice involvement or services not within Hospice Period. No Medicare Payment can be made.
