CMS has posted its January refresh of Care Compare, including new data elements in the Home Health Quality Reporting Program.
 
Separately, CMS has also published the first agency-level data for the expanded Home Health Value-Based Purchasing (HHVBP) model.
 
Care Compare now includes the Discharge Function Score, publicly reported as “How often patients were at or above an expected ability to care for themselves and move around at discharge.”
 
The national average is 67.15%. Agency results are published on Care Compare with the national and state averages.
 
The two other measures added included transfer of health information to the patient and to the physician, published as “How often the home health team reviewed and provided a medication list to the patient, family, and/or caregiver at final discharge” and “How often the home health team reviewed and provided a medication list to the next healthcare setting.”
 
All three of these measures currently reflect data from April 1, 2023, to March 31, 2024.
 
HHVBP results were published in the provider data catalog and includes agency payment adjustments for 2025, along with a lot of other agency results on the HHVBP measures. This is expected to be updated annually each January.
