You can now access a list of the 2,217 hospitals participating in CMS’ Comprehensive Care for Joint Replacement Expanded (CJR-X) Model starting in January 2028.

The CJR-X Model will hold hospitals accountable for costs and care for 90 days after a lower extremity joint replacement, including the patient’s transition into home health. If total spending on the patient exceeds CMS’ target price, the hospital will be penalized. If the cost is below the target price, the hospital will be rewarded.

Procedures included in the CJR-X Model include:
  • Major Hip and Knee Joint Replacement or Reattachment of Lower Extremity with or without Major Complications or Comorbidities (MCC)
  • Hip Replacement with Principal Diagnosis of Hip Fracture with or without MCC
  • Total Ankle Replacement
  • Total Hip Arthroplasty
  • Total Knee Arthroplasty
CJR-X participation will be mandatory nationwide. However, 700 hospitals already participating in the Transforming Episode Accountability Model (TEAM) will be excluded. Notably, there are no participating hospitals located in Rhode Island.
